Below are the steps we take to attain the best care solutions possible for you and your loved ones.
Step 1: Case Management Evaluation
Since every situation is unique, every consultation is unique. We gather the information that can help us find the right plan of care for you. The following are discussed:
- Medical history
- Current cognitive and physical status evaluation to provide a baseline
- Inventory of assistive devices currently used
- Activities of daily living (ADLs and IADLs) to identify any challenges to aging in place as well as to learn the preferred routine
- Prices and care plans
After the consultation, a care manager will provide you with recommendations and help you write and execute a care plan if you wish.
Step 2: Care Plan
When all input and information is collected and reviewed, our caseworkers will present a recommended care plan built around the needs and requirements of the primary users and their support system. We will provide details for the types of services. This plan will be shared with and approved by all necessary parties.
Step 3: Caregiver to Client Matching
After the care plan is composed, a caregiver is carefully selected to tailor the needs, expectations, and personality.
In our experience, hiring a caregiver with the right qualifications is not enough. Matching caregivers to clients is the key to a successful client-caregiver relationship. We believe that the ‘chemistry’ and rapport a caregiver builds with are instrumental to the well-being of the client and the caregiver.
This prevents a high turnover, client satisfaction, and possible depression on the part of both parties.
Step 4: Periodic Follow-Up
Shared information is the foundation for everything that we do at Bloom Homecare. We are committed to keeping each other well informed of your activities and progress, and your feedback shapes how we improve our services and introduce innovations in caring for our clients.
Follow-up visits are to ensure that both the client and caregiver relationship is conclusive. Most importantly, the client is satisfied with the care that they are receiving. This is also a time to change caregivers if a client is not satisfied with a caregiver. Please note that caregivers can be changed at any time before or after a follow-up visit.
